1. I have received a copy of the Constitution and Bylaws of the Jubilee Hospitality Association Canada (“JHAC”) and have had opportunity to review same.

2. I shall comply with such membership Terms and Conditions, Code of Conduct, Constitution and Bylaws of JHAC which may be in effect from time to time.

3. I am a qualified owner as defined in the constitution and Bylaws of JHAC and confirm that I have received all such corporate or other authority as required to seek membership in JHAC.

4. I will keep confidential all terms and conditions of any and all supplier negotiations, contracts and arrangements, including but not limited to pricing, volumes and rebate terms, that are negotiated or contracted by the JHAC and any supplier or third party for the benefit of JHAC and/or its members.

5. I consent to the processing and use of my personal and business information for purposes related to the JHAC, including legal, administration, management, negotiation of procurement opportunities, marketing and analysis purposes.

6. I acknowledge and agree that JHAC may, from time to time, receive rebates, discounts, or other allowances from suppliers in respect of my and other members’ purchases from such suppliers which the JHAC shall be entitled to retain for its own use and credit without accounting to members.

7. JHAC may provide on its website, via email or in publications certain membership application forms, registration forms, order forms, and other requests for information. Such requests may require me to provide contact, demographic and other information. JHAC may use information from such forms to contact me, send me information about JHAC as well as to provide information to third parties in order to negotiate agreements and deliver products and services. JHAC shall not provide my email address, telephone numbers or facsimile numbers to such third parties without my permission.

8. I acknowledge and agree that the JHAC may, from time to time and at its sole discretion, amend membership terms or revoke membership, and that admittance into membership and programs are at the sole discretion of JHAC.

9. I have had the opportunity to seek independent legal advice and have either received such advice or have chosen not to seek same. □ I consent to the sharing of my email address, telephone numbers and facsimile numbers with selected third parties, such as suppliers, so that they may provide me with further information of any contracts and arrangements made by JHAC. □ I consent to the publishing of my contact information on a contact list which may be made available to members or other third parties.
